For customers needing to ship product in for service, we do require you include, with your product, an initialed-and-signed copy of our Repair Contract.
PLEASE NOTE: To view/print the Repair Contract document you must have Adobe Acrobat Reader installed on your computer. If you do not have Adobe Acrobat Reader installed on your computer, you can get it by clicking here.
After you have printed the Repair Contract, please read carefully the Instructions below, and fill in the necessary information. Then, include the Repair Contract with your ship-in product, along with your sales receipt or warranty-coverage information, if your repair will be covered by your manufacturer or a third party. For non-warranty repairs we do charge a diagnostic or minimum fee. Call for more information: (281) 444-0287. NOTE:
Unless you have experience with product shipment we recommend you have your product professionally packed at a UPS or FedEx store. During transit improperly packed items can easily be damaged. If you pay the UPS store to pack it, they will be responsible should any shipping damages occur. While it costs more to have products professionally packaged, we feel it is worth the effort. Nothing is more frustrating (for you and for us) than to fight UPS or FedEx over a shipping claim.

Please fill in all the contact information fields, along with a thorough description of your symptoms. If necessary, include a print out or note of the symptoms if you need more space.
For all warranty units please initial the box about halfway down the page, and also sign, in the Signature field towards the bottom of the invoice.
If the unit is out of warranty, please just sign in the Signature field. For out of warranty units we will contact you to make arrangements for payment of the estimate fee. Please pack your unit well and insure it for the value you desire. Please list the amount you would like us to insure it for when we ship it back to you. Put this amount in the Insurance field above your signature. We will contact you once the diagnosis has been made. Please be sure to include any and all necessary and useful contact information, as well as a clear written description of the problem with your unit.
