PLEASE READ IN FULL BEFORE HITTING “PRINT”!

For customers needing to ship product in for service, we do require you include, with your product, an initialed-and-signed copy of our Repair Contract.

PLEASE NOTE: To view/print the Repair Contract document you must have Adobe Acrobat Reader installed on your computer. If you do not have Adobe Acrobat Reader installed on your computer, you can get it by clicking here.

After you have printed the Repair Contract, please read carefully the Instructions below, and fill in the necessary information. Then, include the Repair Contract with your ship-in product, along with your sales receipt or warranty-coverage information, if your repair will be covered by your manufacturer or a third party. For non-warranty repairs we do charge a diagnostic or minimum fee. Thank you for your interest in shipping us your electronic unit for repair service. Nothing is more frustrating (for you and for us) than to fight UPS or FedEx over a shipping claim. Freight shipping on bulky or heavy, somewhat fragile electronic items can be tricky. In our experience UPS will deny insurance claims without proper packaging. For maximum protection, and to safeguard your equipment during its transport to us and back. we advise first, to get a good, sturdy, double-walled box. Wrap item in a garbage bag or plastic sheeting if you will be using any Styrofoam packing materials. (When a box is mishandled shards of Styrofoam enter the unit’s cabinet through the vents causing possible damage).

If this unit is a reel to reel, film or a slide projector, take the reels off and keep, we have testing items for these accessories and don’t require them.

Please secure accessories like power cords, or other objects separate from the unit, and make note on your
ship-in form.

During transit improperly packed items can easily be damaged.We choose to use heat-expanding foam, but it’s somewhat expensive; we use Sealed Air Instapak Quick RT, which can be bought through Amazon, Ebay, and some office supply and businesses that specialize in packing and shipping like a UPS or FedEx authorized location. This packing material
can be re-used with the same unit and box for return shipment.

This is the most protective option, using it is a bit tricky but they have a tutorial video online; basically you crush
the chemical pack and the foam heats and expands, into any available space in your box. We typically use
4 or so per item shipped.

Short of that, wrap the bagged unit THOROUGHLY with bubble wrap, and situate deeply in a double-
walled box full of packing peanuts or other dunnage. Pack the box TIGHT. Slippage around inside is theenemy, beyond any rough handling by UPS. It should survive shipment; however, IF YOUR PRODUCT IS SHIPPED TO US IN A NON-SPEC BOX/PACKING, we may require box/packaging upgrade to return it to you (alternatively you may waive the upgrade, if you grant us permission to return it to you with no liability on our part.)

Alternatively, you can take it to a UPS or FedEx store and let them pack it, they usually don’t follow
spec, but they’d be responsible in the event of damages. Insure to your desired value.

If a participating The UPS Store® location packs your item using only new materials purchased
at that participating location, and ships the package via UPS, USPS, or DHL (on that
participating location’s applicable UPS, USPS, or DHL account) and that item is damaged or
lost in transit, you’ll be reimbursed:

The item’s value (subject to the lesser of actual value, replacement or repair cost)
• The full retail price you paid for packaging materials and services
• Your cost for shipping (excluding Declared Value charges or costs for Insurance
as applicable)

For UPS shipments, if you do not declare the value of the item at the time of the transaction and
purchase Declared Value insurance for that amount, your maximum reimbursement for the item’s value is
$100.

Last, but not least, please include a filled in and signed copy of our Ship-In Repair Contract.  Also, you may include your initial diagnostic deposit quoted through contact with us. A check with your shipment will be deducted from your total repair quote, after we alert you of our technician’s findings and proposed repair plan; alternatively we can contact you for credit card payment of our minimum charges on your product. We will communicate with you upon receiving your item for repair, and inform you of your in-house ticket number.

If this will be a warranty repair, please send in a copy of your original sales receipt with the unit. Upon receipt one of us will
contact you with your service number.

 

Here are the instructions for filling in the Repair Contract Form:

Please fill in all the contact information fields, along with a thorough description of your symptoms. If necessary, include a print out or note of the symptoms if you need more space.

For all warranty units please initial the box about halfway down the page, and also sign, in the Signature field towards the bottom of the invoice.

If the unit is out of warranty, please just sign in the Signature field. For out of warranty units we will contact you to make arrangements for payment of the estimate fee. Please pack your unit well (see above) and insure it for the value you desire. Please list the amount you would like us to insure it for when we ship it back to you. Put this amount in the Insurance field above your signature. We will contact you once the diagnosis has been made. Please be sure to include any and all necessary and useful contact information, as well as a clear written description of the problem with your unit.
